Admission Overview

Vila Monte will accept and retain up to 32 residents age sixty and above.

  • Residents under the age of sixty may be admitted if they meet the guidelines outlined in our Admissions Policies.
  • Residents determined by their physicians to have a primary diagnosis of mental disorder, unrelated to dementia shall not be accepted or retained.
  • Up to 32 residents may have a non-ambulatory status.
  • Of the 32 residents, up to 4 residents may be bedridden at any one time.
  • Of the 32 residents, up to 4 residents may receive hospice services at any one time.

Admission Policy
 
Vila Monte is a non-medical facility. Vila Monte does not discriminate on the basis of race, religion, creed, color, or actual or perceived sexual orientation or ancestry.  It is the purpose and goal of Vila Monte to provide optimal care to seniors over the age of sixty (see exception below in Admission Criteria #1) who are in need of non-medical care and supervision. Prior to admission the resident or responsible party will give permission to obtain medical information, in writing, so Vila Monte can gain a clear understanding of the individual needs of the resident.

Admission Criteria

We shall accept up to 32 residents. Up to 32 residents may be non-ambulatory. All residents must meet the following criteria for admission:

  1. Each resident should be at least sixty years of age (only one resident under the age of sixty may reside in the facility at any one time if their needs are compatible with the other residents in the facility).
  2. Every resident must have a pre-appraisal carried out by the administrator, a physician report and TB test completed indicating no communicable disease (i.e. tuberculosis) prior to admission.


    Applicants with the following conditions will not be considered for admission:

    • Those requiring complete assistance with all activities of daily living.
    • Those with a prohibited health condition.
    • Residents determined by their physicians to have a primary diagnosis of mental disorder, unrelated to dementia.
    • Gastrostomy tubes, tracheostomy, nasogastric tubes, IV’s.
    • Infectious disease.



    Applicants requiring in patient care in a health facility.

    • Oxygen tents.
    • Un-managed incontinence.
    • Disabling contractures.
    • Diabetics with metabolic instability.
    • Wounds requiring irrigation.
    • Stage 3 & 4 dermal ulcers.

    The following conditions are allowed only if the person is 100% able to care for the condition without assistance or assistance is given by a skilled medical professional.  Facility staff may not assist with these conditions.

    • Oxygen administration
    • IPPB Therapy
    • Colostomy or Ileostomy
    • Enema, Suppository and/or fecal impaction removal
    • Contractures (non-disabling)
    • Diabetes
    • Injections (IM or SubQ)
  3. When an incidental medical need exists, admission will be considered when there is close supervision and care by a licensed home health agency.
  4. Prior to admission, each resident and/or responsible party will tour Vila Monte and disclose all pertinent information related to the resident which will allow for optimal care.
  5. All applicants must be free from severe mental illness and/or behavior problems.
  6. All physician medication and care orders must be received prior to admission.
  7. Residents will be expected to have the capability to dine in the dining room, except for temporary illness.
  8. Residents will be expected to self-administer medication, which will be centrally stored.
  9. The following are examples of conditions which are considered appropriate for placement:
    • Use of oxygen when blood gases are stable and the resident is capable of self-administration.
    • Colostomy, when the resident is able to manage all aspects of the condition.
    • Ileostomy, when the resident is able to manage all aspects of the condition.
    • Managed incontinence (both bowel and bladder).
    • Stage I and II decubitus ulcers.
    • Post-surgical wounds when the wound is well approximated.
    • Diabetes including insulin dependent providing the resident has reasonable stability, and is able to self-test and self-inject.
    • Inhalation therapies.
    • Dementia
    • Mild cognitive impairment.
    • Mild depression.
    • Up to 32 residents may have a non-ambulatory status.
    • Up to 4 residents may be bedridden.
    • Up to 4 residents may receive hospice services.
  10. The criteria for determining placement will be:

    • Interview with resident by the administrator.
    • Interview with family members, or responsible party by the administrator.
    • Interview with any appropriate home health care agency team member by the administrator.
    • Interview with involved social worker by the administrator.
    • Current physician’s report/medical assessment/TB clearance completed within one year prior to placement and submitted prior to admission
    • Completed preadmission appraisal plan completed by the responsible party/resident with accompanying interview by the administrator
    • Completed emergency information sheet completed by the administrator and responsible party/resident.

Conditions that would require resident relocation:

  1. Prohibited Health Conditions:
    • Gastrostomy tubes, tracheostomy, nasogastric tubes, IV’s.
    • Infectious disease.
    • Applicants requiring in patient care in a health facility
    • Oxygen tents
    • Un-managed incontinence
    • Disabling contractures
    • Diabetics with metabolic instability
    • Wounds requiring irrigation
    • Stage 3 & 4 dermal ulcers
  2. Residents determined by their physicians to have a primary diagnosis of mental disorder, unrelated to dementia.
  3. Those requiring complete assistance with all activities of daily living.
  4. Suicidal ideations.
  5. Unsafe behavior that may pose a hazard to the resident or others.
