Our Services

All services provided by People's Care are diligently supervised by a People's Care Professional Care Manager. The Professional Care Manager is a social worker or nurse and an employee of People's Care. He or she is your main point of contact throughout the entire care Process. From conducting your free, in-home comprehensive Care Assessment, to adjusting your loved one's Individual Service Plan (ISP) as the need for care changes, your People's Care Professional Care Manager is with you every step of the way.

Comprehensive Care Assessment

During this free, in-home comprehensive Care Assessment, your People's Care Professional Care Manager's goal is to fully understand your loved one's physical, social and cognitive needs - as well as your own concerns. He or she will gather the following information into one document:

  • Family history
  • Health considerations
  • Current medications
  • Daily care needs
  • Social and recreational needs
  • Behavioral considerations

This comprehensive Care Assessment may also include a home safety inspection and cognitive screening, as needed. The assessment process requires approximately one to two hours.

Individual Service Plan

Our Professional Care Managers will provide you with an Individual Service Plan (ISP) that focuses on your loved one's health and well-being, as well as his or her social and recreational needs, and can be customized to include the following:

Comprehensive Care Assessment

We provide this service for free. Our nurse or care coordinator will evaluate your health conditions and recommend a care plan that will address the areas where you need assistance.l

Care Management Supervision

A Professional Care Manager from Peoplescare In-Home Care will match one or more caregivers from our Care Companions. The Care Companion(s) will have the skill set that enabes him or her to efficiently address your health, cognitive and social needs.l

Attending Physician Appointmentsl

Your Professional Care Manager will attend physician's appointments with you - providing you with emotional support during the examination, consultation or medical tests. After the physician's appointment, your professional care manager will report back to your family members upon request. This ensures continuity of care as prescribed for you by your physician.
